This squid salad is called Ika Sansai and is served by many sushi restaurants and Japanese grills. I have never been able to find a recipe for it and it appears that most Japanese restaurants buy it frozen from a Japanese supplier instead of making it themselves. You can buy it frozen in most asian markets. That’s where i get mine. I haven’t tried making any yet, but I’m considering it since I have a list of ingredients to work from.

Here is the list of ingredients: squid, vinegar, kikurage mushroom, ginger, sugar, yamakurage (lettuce stem), bracken fern, sesame oil, soy sauce (soybean, water, wheat flour, salt) bamboo shoot, sesame seed, salt, bonito extract, kelp extract, red pepper, sweet rice wine, rice wine, high fructose corn syrup, xanthan gum, annatto.

I imagine that with some trial and error you will be able to come up the the correct combination of ingredients and measurements. Send me a recipe if you come up with a good one. Good luck!
